U.S. News & World Report has published its 2025 rankings of the top U.S. rehabilitation hospitals. A total of 1,049 facilities were evaluated based on patient outcomes, discharge and readmission rates, specialized programs, staffing, and rehabilitation technology.
To establish performance standards, data from all facilities were analyzed, and hospitals were scored according to how they compared to these metrics. Facilities that exceeded national averages received higher scores, which—along with expert opinion—determined their overall rankings.
Notable national outcomes measures for 2025 include:
- Prevention of hospital readmission after discharge: 93.1%
Percentage of patients who were not readmitted to an acute-care hospital within 30 days after discharge.
- Prevention of hospital readmission during rehabilitation: 95.6%
Percentage of patients who did not require transfer to an acute-care hospital during their rehabilitation stay. - Patients discharged home: 65.4%
Percentage of patients discharged directly home rather than to another facility—an indicator that the patient was well enough for home-based self-care.
