When should a child transfer to an adult rehabilitation program?

The transition from a child-centered rehabilitation program to an adult-centered needs to take place over time, and needs to be prepared for. So, by the age of 21, they should have established an adult-centered care center facility that they want to go to, a primary care physician, a urologist, maybe a hand surgeon, whoever else you need on your team. And, communicate those resources to all of those specialists in the child-centered care realm, so that there's a continuity of care as you transition. But somebody that your child, young adult, feels comfortable with is most important. Because the key is that they're going to be taking over their own health care decisions. And so, they need to feel comfortable with the person that's going to be working with them.
